Police arrested a man who left a suicide note at his wife's house and charged him with harassment days before he took his own life, an inquest has heard.

Father-of-five Shaun Smith was found dead in the Cley village hall car park on July 2 last year. He was 35.

But an inquest into his death heard that two days earlier he had been charged with harassment after his wife had phoned police fearing for his welfare after he left a suicide note outside her home.

In a short statement read to Norfolk Coroner's Court, his partner of 16 years and wife of nine said: "Health care and police failed him on more than one occasion."

Laura Hines, his sister, told the court: "He was a father, brother, uncle, great uncle and friend and he deserved better.

"He was let down so badly."

The court heard Mr Smith suffered with depression and anxiety, while also battling post-traumatic stress disorder after being attacked as a teenager.

In the months leading up to his death he had been prescribed with antidepressants and was receiving cognitive behavioural therapy - but had been discharged from wellbeing services less than a month before he died.

Meanwhile, the court also heard allegations that he been bullied over his mental ill health while working at Booker wholesalers in Holt - although he had left the company and found a new job as a security guard by the time of his death.

Area coroner Yvonne Blake concluded his death was suicide, with the medical cause given as carbon monoxide poisoning.

She said: "He did seek some help but he did not carry on with it perhaps as long as he should have done.

"When his wife called the police she had done so to try and get him help 

"He clearly felt he could not see any other way out."
