Further details have emerged of the hospital visit made by a father who was later found dead alongside three members of his family.

Bartlomiej Kuczynski died on Friday at his home on the Queen's Hills estate, Costessey, along with his two daughters, Jasmin and Natasha, aged 12 and nine.

The girls' aunt, 36-year-old Kanticha Sukpengpanao, was also found dead inside the property in Allan Bedford Crescent, with the circumstances of the tragedy under continuing investigation.

Part of the investigation centres on the way an earlier incident, a month earlier, was handled by police and the health services.

On that occasion, on December 14, Mr Kuczynski was reported missing.

He was later found on the Queen's Hills estate close to his home and taken to the Norfolk and Norwich University Hospital by police officers so he could undergo a mental health assessment.

However, the 45-year-old left the site in Colney before the full assessment could be undertaken.

Now, further details of just what happened on December 14 have been revealed by the hospital.

It has emerged Mr Kuczynski did receive a comprehensive review while being triaged by staff - but left before follow-ups could be carried out.

This meant he was not formally discharged from the hospital, which does not have specialist mental health facilities, before leaving.

A spokesman for the NNUH said: "We can confirm that the patient was taken to the emergency department by police on December 14. 

"Following a comprehensive clinical assessment, the patient was assessed as having mental capacity and he left the unit before being reviewed again by the team.”

The hospital trust itself does not provide mental health services, with this responsibility lying with the Norfolk and Suffolk NHS Foundation Trust, but its A&E facilities can be used by NSFT for urgent mental health assessments.

The incident, which saw police leave Mr Kuczynski in the hospital's care, has sparked a referral to the Independent Office for Police Conduct.

The watchdog is also investigating a 999 call made by Mr Kuczynski an hour before the bodies were found, last Friday morning.

He told call handlers he was confused and raised concerns about his mental state, but no officers were sent to the house.

Meanwhile, NSFT is facing calls for greater transparency over its dealings with the structural engineer in the months leading up to the tragedy.

Neighbours have said Mr Kuczynski had been struggling with mental ill health for several months and had been seeking support.
