The death of a four-year-old boy in a stair gate accident is due to be investigated for a second time.
Draco Chapman, of Lakenham, Norwich, died on April 23, 2019, after suffering "irreversible and catastrophic brain injuries" in an incident involving a stair gate.
An inquest into his death held in January last year concluded that it was an accidental death.
However, this finding has now been overturned and a second inquest into his death scheduled.
During a pre-inquest hearing held on Monday, senior coroner Jacqueline Lake confirmed that a fresh inquest will be conducted later this year.
Following the original hearing, Draco's father David Chapman said that his family had been "destroyed" by his son's traumatic death.
A serious case review was then published by Norfolk Safeguarding Children Partnership.
This report stated concerns were raised by teachers and health visitors months before he died.
Mrs Lake adjourned the hearing for a further review on April 13.
The second inquest into the youngster's death is then scheduled to start on October 23.
