James Maddison will undergo scans in the next 24 hours to assess the extent of a left knee injury that capped a miserable 5-1 final day Championship defeat at Sheffield Wednesday.
City's player-of-the-year lasted just nine minutes at Hillsborough before walking straight down the tunnel and returning at the end of the game on crutches to applaud the travelling support.
The highly-rated midfielder is being strongly touted with a Premier League move this summer, but head coach Daniel Farke insisted the potential fallout can wait.
'I am not worried (about whether he has played his last game). I am worried about his health,' he said. 'I am concerned that he has suffered a big, big injury in this final match. We don't want him to be out for several weeks - hopefully not months, and the summer break is not enough for him to return.
'Let us not get too far ahead of ourselves.
'We have to wait. Not good. There is something with the knee. There will be a scan tonight or perhaps tomorrow morning. We must wait for the results.
'It could be ligaments or the inner band. He got a hit on his knee and he felt it break a bit. It doesn't look good.
'It was a duel and he slipped away from the tackle and felt something in his knee. If I am honest, it doesn't look good at present and he doesn't feel happy with his knee.
'Let's wait what the scans say.'
Christoph Zimmermann was forced off after suffering a facial injury in the build up to the Owls' fourth goal while Ivo Pinto - who replaced Maddison in the 11th - picked up an Achilles problem in the final seconds of the opening period.
'I should have substituted Ivo as well. He had an Achilles injury and it was a big risk for him to play on but we had so many other injuries,' said Farke.
'I am not sure if Christoph collided with Angus (Gunn) or an opponent. I would have to watch it back. We were concerned he had broken something in his face. He played further on but felt some headaches and weakness in his legs. We had to be careful then so we substituted him.'
