A coroner has made a series of recommendations following the death of a 36-year-old Norwich woman.

Rita Lita was found at her home on Hall Road, Norwich, on September 13, 2017 having hanged herself and consumed high levels of alcohol.

An inquest in Norwich on Monday heard how Mrs Lita, originally from Lithuania, had a history of depression, alcohol abuse and self harm, and had recently split from her husband.

Coroner Yvonne Blake said there were two 'lessons learned' from the 'sad and unhappy' case.

She said all key workers at the Norfolk and Suffolk NHS Foundation Trust should make contact with all patients at least once a month, if not in person then over the phone.

The other lesson is that individual practices should be dealt with by the key workers's line management.

The inquest heard that Mrs Lita was seen by various mental health services, but had never met her GP.
